Verify you've submitted your medical forms and completed the parent survey.

To provide copies of your medical forms:

Download copies of blank Physical Form and Medication Forms from the Medical Forms folder in Files on BoosterHub.

Once the forms are completed and signed by your student's doctor, upload a pdf or photo of the forms below, email them to Secretary@pcmb.net, or drop a paper copy in the white mailbox in the band room.

If you choose to upload the forms below, please include your student's first initial and last name and brief description of the form in the filename. (Example: JSmith-Physical Form.pdf). You can upload both pages of the MHSAA Physical Form as one file or each page as a separate file if needed. Please ensure both pages are uploaded before submitting.

To complete the parent survey:

Access and complete the Parent Survey. The survey allows us to better understand interest levels for volunteer opportunities, committees, and areas where families may want to get involved. Complete at least one parent survey per family, but each parent may submit a separate survey if you wish.
